Summary of Benefits for Flexible Staff Employees
Semper offers a complete and competitive benefits package to our qualified workers who work directly for us. (some classes of workers are excluded please contact HR to determine your status 800 954 4993) All rates and benefits are subject to change. Call the Corporate HR Department for current information. The benefit package can change without notice. This package includes:
Health Insurance Package
Enrollment is once a month (beginning of the month). You must work 240 hours in the previous two months. If you qualify, your share of the monthly premium will be deducted from your paycheck to cover the following month. Effective July 1st, 2010 the employee contribution per week for a Single PPO Plan: $53.84, for employee plus one $144.95, for a family $230.33. (For MA residents only a choice of HMO Plans is also available: single $62.71 or $55.44, employee plus one $161.85 or $147.86, family $265.51 or $234.84). Semper will contribute part of the premium as long as you meet eligibility and the hours requirement (240 hours in the previous two months on a continuing basis). The cost covers all health benefits. The benefits include all of the following:

- LTD (Long Term Disability) - Assurant
- Life and AD&D Insurance - Assurant
Section 125 Cafeteria Plan
Pay your insurance premiums with pre-tax dollars and reduce your taxes.
Section 132 Parking and Transportation pre-tax deduction Plan
All employees are eligible to deduct qualified parking and transportation expenses
on a pre-tax basis. Enrollment is once a month (beginning of the month).

HSA Plan (Health Savings Account)
Semper's PPO plan is HSA compatible. Opening an HSA allows you to save pretax dollars to pay qualified medical expenses including but not limited to medications, office visists and lab tests. Semper's HSA plan is admnistered by Wells Fargo. Please call Human Resources in Boston for more information. (Please note the HSA is not available to HMO participants.)
401K Program - Smith Barney
Enrollment is quarterly. You must have worked 500 hours in the previous six months. You can choose between 1 and 15% of your pay to contribute to your plan. The provider is Smith Barney. Semper matches are temporarily suspended. New participants can roll over assets from former qualified plans. Employer contributions made prior to April 28,2008 follow a vesting schedule based on years of service. One year of service is 50% vested. Two years of service is 100% vested.
Holiday Pay
Eligibility is 1800 hours in the previous 52 week period. Employees must work the week of the holiday. Semper issues a check for $75. Holidays observed are New Years, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas.
